✅ Included Services A typical Health Assessment Session may include: 1. Vital Signs Check • Blood pressure • Heart rate • Respiratory rate • Temperature • Oxygen saturation (SpO₂) 2. Blood Glucose Screening (CLIA-waived) • Fasted or random blood sugar test using fingerstick • Client education based on results 3. BMI & Weight Assessment • Height, weight, and BMI calculation • Discussion on healthy ranges 4. Health Questionnaire Review (Optional) • Lifestyle, stress, diet, and sleep habits • Health goals or medical concerns • Recommendations to follow up with primary care 5. Cancellation Policy

Deposit & Payment Policy All sessions require a non-refundable deposit to secure your appointment. Deposits go toward your total cost. Cancellations or rescheduling made less than 24 hours before the session will forfeit the deposit. No-shows may be required to prepay in full for future appointments. ⸻ Service Pricing Pricing varies based on services included. Standard rates and any add-ons (e.g., rapid tests, additional screenings) are listed on the Services Page and confirmed at booking. All rates are subject to change without notice. ⸻ Travel Fee Policy Mobile sessions may include a travel fee, depending on distance and location. This fee will be disclosed during scheduling. ⸻ Client Requirements • Present a valid government-issued ID if requested • Complete a brief health intake form • Provide honest and accurate health information • Be in a suitable space for screening (private, clean, well-lit) ⸻ Results & Education Clients will receive a summary of results and general health education. Any abnormal results will be accompanied by a recommendation to follow up with a healthcare provider. Gifted Aura does not diagnose, treat, or prescribe. ⸻ Confidentiality & HIPAA Compliance All information collected is protected under HIPAA guidelines and will not be shared without written consent. Clients may request a digital copy of their results via secure email. ⸻ Right to Refuse Service Gifted Aura reserves the right to refuse or end service if: • The client is uncooperative or inappropriate • The environment is unsafe or unsuitable • The requested service exceeds CLIA-waived testing scope ⸻ Refunds & Rescheduling No refunds are issued for completed sessions.
